Care Management
At Community Vision, every Support at Home customer is partnered with a dedicated Community Life Manager. They’re your main contact and will guide you through your care journey.
The government sets aside a 10% of your Support at Home funding to cover care management. This amount is separate from the budget you use for your regular services. We use that funding to cover the time we spend supporting you.
But don’t worry — this funding doesn’t limit the care you receive. You’ll get as much care management as you need, no matter what. There’s no cap, and no extra cost to you.
You won’t need to pay anything extra for care management. It’s fully covered — there are no out-of-pocket costs. Fees are calculated in 15-minute blocks, following Services Australia’s guidelines.

Contributions for Support at Home
Under the Support at Home program, you may be asked to contribute to the cost of your services. This contribution is based on what you can reasonably afford and is determined through an Income and Means Assessment conducted by Services Australia. The amount you pay will fall within limits set by the government and reflects your financial circumstances, including your income and assets.

If you have been assessed to contribute toward your Support at Home services but are experiencing financial hardship, you may be eligible for financial hardship assistance.
