Support Service Coordination
Independent living refers to the services you can access to help you gain confidence and learn life skills in your own home and out in your community, so you can live a full and happy life with the confidence that you have support if you need it.
Our team is dedicated in supporting you to live your life, your way. With over 45 years’ experience we make sure all our staff are fully qualified with the expertise they need to make sure you keep receiving the very best support.
All our support services perform strongly against the NDIS Practice Standards and are measured by an independent third party. This means we meet the quality standards that apply to the disability sector, and that our staff meet all requirements to ensure your safety.
What is the National Disability Insurance Scheme (NDIS)?
The NDIS is the National Disability Insurance Scheme. The NDIS gives you the freedom to choose who supports you, and the flexibility to work with who you choose as you service provider to help you reach your goals.
How do you apply for NDIS funding?
Step 1 – Check your eligibility: Call 1800 800 110 or email email@example.com to find out if you’re eligible for the NDIS.
Step 2 – Make a request: Make a Verbal Access Request (VAR) or fill out an Access Request Form (ARF). You can find the form on the NDIS website or call 1800 800 110 or email firstname.lastname@example.org.
Step 3 – Receive your access decision: The NDIA will write to you to confirm your eligibility. If you’re eligible, you’ll be contacted to arrange a planning meeting.
Step 4 – Plan your support: Give us a call 1800 968 754 or email Enquiries@communityvision.asn.au and we’ll work with you to plan and provide your NDIS support, so you can live your life, your way.
We can provide guidance every step of the way ensuring you have complete understanding and clarity about type of supports your funding can provide. In doing so it will give you the flexibility and choice to decide what supports you want to meet your plan goals.
